CHPS Criteria Interpretation Form

Do you have a question about the CHPS Criteria? Use the CHPS Criteria Interpretation Form to get your question to CHPS staff. Some questions will have easy answers; other questions will require a formal Criteria Interpretation. Formal CHPS Criteria Interpretations are available free of charge for CHPS Verified registrants (up to 3 per project). If your project is not registered with CHPS Verified, formal Criteria Interpretations will cost $250.00 each (click here for a payment form), or $200.00 if you are a CHPS member. CHPS staff will alert you before the interpretation process begins if your question qualifies for a formal review.
